Complete Adult Physical

Please select Complete Adult Physical when you are scheduling an annual exam, which may include an annual pelvic or pap smear exam, if indicated. Reminder: Physicals are a health maintenance exam.

Well Child Check (which are also used for Camp/Sports Check Ups)

Please select Well Child Check when scheduling a routine visit for your child. Routine visits are recommended at the following ages:

• 2 weeks
• 2 months
• 4 months
• 6 months
• 9 months
• 12 months (This visit needs to be on or after your child's birthdate)
• 15 months
• 18 months
• 2 years
• 3 years
• 4 years (This visit needs to be on or after your child's birthdate)
• Yearly visits after the 4 year visit

*If you have had a Well Child Check in the last 12 months and need a Camp/Sport Check-up form filled out, no appointment is needed. Please mail your forms to your clinic.*

Office Visit

Please select Office Visit when you have a specific non emergency issue, such as:

• ADHD
• Diabetes
• High blood pressure
• Thyroid
• Cardiac
• Asthma
• Depression/Anxiety
• Pain Management
• Medications
• Follow up
• High cholesterol
• Respiratory infection
• Sore throat

**An Office visit is not intended to be used for scheduling a Complete Physical. If this is a new illness or you have questions about whether you or your child needs to be seen immediately, please call your clinic.**

Pregnancy Care

This appointment is for care during your pregnancy after your first visit. For your first pregnancy visit, please call the clinic to schedule your appointment.

E-visit

Please consider receiving care online from the convenience of your home or office for the following concerns:

• Acne
• Burns (minor)
• Canker or cold sore
• Cold, sinus infection, or sore throat (upper respiratory infection)
• Constipation, diarrhea and/or irritable bowel syndrome
• Diaper rash
• Eczema or dermatitis
• Female bladder infection (UTI)
• Hay fever/allergies
• Influenza (the flu)
• Lice
• Lower back pain
• Pink eye (conjunctivitis)
• Shingles
• Stye (bump or bumps on the eyelid)
• Tick bite
• Tinea (fungal skin infection)
• Tobacco cessation
• Vaginal yeast infection

Insurance Considerations

You may want to check with your insurance company to ensure the appointment you are scheduling will be covered under your policy. For example, if your child is over the age of two or if you are an adult, some insurance companies will not pay for routine physicals that are scheduled less than twelve months apart.
